Life with HIV/AIDS involves various challenges, such as figuring out medical care and coverage, applying for social/community based services and educating yourself about HIV and how to prevent transmission.  These are issues that you don't have to face alone.  GMAP/MVAP provide Case Management Program that is available to anyone living with HIV within the Greater Manchester and Concord area.  GMAP/MVAP's Client Services Department has years of experience in supporting clients, their families, partners, and friends.

Becoming a client of GMAP/MVAP begins with a telephone call or drop in visit.  The Case Managers are the first point of contact for all clients.  Together, the client and case manager will develop a service plan based on client-identified goals.

Intakes are conducted at our office.  In certain cases, case managers can meet clients at hospitals, medical offices, or the client's home.  During the intake, you and your Case Manager will have the opportunity to discuss issues concerning your health, financial resources, insurance, housing, mental health, and support systems.  Your Case Manager will work with you to address any areas of concern that you might have, answer questions, and provide support.
